Help our patients with life-limiting illnesses to spend Christmas and the New Year in the comfort of their own homes, supported by the palliative care of our specialist nurses. Each year we provide 24/7 Hospice at Home care, for over 250 people which is free to patients but has a cost of £291,000.

    Situation

    Our Day Hospice provides services delivered by dedicated, caring staff, but it's true that ‘there is no place like home’, particularly during the festive season. The main reason why some of our patients are unable to spend Christmas at home is a lack of appropriate community healthcare support. Despite families caring for their dying relative, they do not have the specialist knowledge and skills and need practical and emotional help to cope with the strain of 24-hour care.

    Solution

    Our dedicated Hospice at Home team of Registered Nurses and Health Care Assistants provides access to services, 24 hours a day, seven days a week, to enable end of life patients to receive palliative end-of-life care and therapies in their own homes and are on call 365 days of the year, including Christmas and the New Year. They also provide support and respite from care for the patients’ carers and families to enable them to enjoy their time together.
