Every hour of every day, our hospice at home nurses visit patients and their families providing essential nursing care and reassurance so patients can remain at home rather than being admitted to hospital. This is even more important at bank holidays and weekends when other services are unavailable.

    Situation

    We are providing out of hours care for our patients and families giving them the reassurance that whether it is day, night, weekends or bank holidays they can call to receive clinical and emotional support. Our families tell us that regardless of which nurse visits, they always know what care has been given previously and that this seamless care is highly valued, taking away the worry and helping keep patients out of hospital.

    Solution

    We have five hospice at home teams and an overnight team providing medical and emotional support for patients and their families. Last December, our hospice at home teams made 242 visits and 1,265 calls at weekends, bank holidays and during the night. You can make a real difference by supporting us so that we can continue to provide our service to those patients who need us. The money raised through this campaign will pay for 10 days of patient care starting from Christmas Eve 24 Dec-2 Jan 2022.
