Situation
In Northern Ireland there are many areas which provide fantastic follow-on programmes for those who have completed Cardiac Rehab in a hospital. However, there is not equitable access to these services across the region. NICHS hopes to fill this gap with the newly developed Heart Strong - Heart Health and Recovery Programme. The aim is to ensure ongoing support, for everyone who needs it and wants it, across Northern Ireland.
Solution
We plan to launch a pilot of Heart Strong, a Heart Health & Recovery Programme, in 2021. Individuals will be referred into the pilot by Cardiac Rehab Teams in the South Eastern Trust. The progamme is delivered in 3 segments Head Strong, Heart Strong and Stay Strong. Each programme will run for 12 weeks allowing 4 cycles a year in the 4 identified areas. It provides additional places for carers of participants who often share the same lifestyle and are at risk of developing heart conditions.
