Sage House: Creating a community dementia hub

We change the lives of people with dementia through joined up personalised support. Services at Sage House help people navigate the fragmented health and social care landscape. We provide trusted, emotional and practical help. The first of its kind, we’re transforming community dementia services.

  • “Lockdown was hell. I was so worried David would unlock all the doors, I didn’t go to bed. By the time we came to Sage House and accessed the Wayfinder service with Louisa, I was at the end. Being able to talk to someone makes a great deal of difference. It’s still hard, but life is more bearable.”

    — Rosemary, who cares for her husband David. They are regular Wayfinder customers

  • “Caring for someone 24/7 is difficult, and Sage House gives us a break. It’s a big relief to be able to take Jean somewhere where the people are so caring and engaging and devoted, it’s a really positive experience for everyone. She comes out full of beans, happy as Larry.”

    — Peter, who cares for his mother-in-law Jean. Jean attends our Day Breaks respite service.

  • “I was in a very difficult place, and when I phoned for help there was a member of the specialist Wayfinding Team straight away, not next week or next month, it was the very next day. And thank god, because I would never have coped without their support. Thank you!”

    — 'Tracey' who receives support from our Wayfinding Team of specialist support workers

  • “When Reece phoned me twice a week in the first lockdown, it was the only proper conversation I had. Peter was a chatty person, but now we can’t really talk. During the second lockdown, Day Breaks is a lifeline. I get some time off from worrying, and Peter gets to see other people.”

    — Angela, who cares for her husband Peter

Dementia is a devastating, constantly evolving condition which has a huge impact on everyone it touches. It is a growing problem and with an aging population, diagnosis rates are increasing. Every 3 minutes someone in the UK will develop dementia, 1 in 14 people over the age of 65 develop dementia, and 1 in 6 people aged over 80. Support for people with dementia varies according to location and is fragmented at best. There is currently no cure and it is the leading cause of death in the country.

Our unique dementia hub, Sage House, transforms how community dementia services are delivered, accessible to anyone, pre and post a dementia diagnosis. Customers are supported by specialist staff with advice on issues like accessing benefits, keeping healthy, managing changing behaviour and providing emotional support to carers. Alongside are inclusive sessional activities for a healthy mind and body and a day respite service. We hope other communities can benefit from this dementia hub model.
