To fund a Clinical Nurse Specialist for 1 year to provide specialist palliative care within the home for patients with life-limiting illnesses and their families. They liaise with GPs and refer to other parts of our service as appropriate, and are often described as the lynchpin of our service.

Situation
In recent years there has been a shift towards providing more palliative care services in the patients’ preferred place of care, in line with the government’s End of Life Care Strategy. For the majority of people, the preferred place of care is their home or care home. Our Clinical Nurse Specialists are involved from the point of diagnosis of a life-limiting illness, and are able to help patients and their families to access the services that they need.
Solution
