1.Poor experience of end of life care by many of the people in East London particularly as a consequence of culturally insensitive and inadequate care 2.Inadequate home based services for people who wish to be cared for and die at home, particularly in relation to practical, social and emotional support for the patient and his/her family 3.Inequitable take up of existing end of life care services 4.Lack of coordination of provision and training by the voluntary sector

Solution

•Recruit, train, guide and supervise volunteers interested to develop services and support people at the end of life. •Development of the Hospice’s community space and information resource as the project hub, establishing this as the local focus for expert public education, information and signposting, advice and support regarding end of life care and bereavement. •Establishment of a group working on local strategy and policy to support this initiative, serviced by the project lead.
