    Situation

    National and internal research shows that carers report a negative impact on their physical and mental health largely due to insufficient practical support. Many patients with life-limiting conditions prefer to remain in their own homes for as long as possible & many give it as their preferred place of death. But without professional support, their wishes can't be met. By supporting the carers and family, we can ensure that carers remain healthy and patients can choose where to die.

    Solution

    The role of the Hospice Clinical Nurse Specialist (CNS) is pivotal in ensuring that the appropriate care and support is received, enabling more families to realise their wish to care for their dying loved-one at home for as long as possible. The team liaise with medical and social care professionals and can call on the services that the Hospice provides to support the patient and carer, including physiotherapy, complementary therapy, counselling, chaplaincy, welfare advice and support groups.
