Hospice transition services

We want to develop the North East’s first Transition Service for young adults with life limiting conditions. We will offer short breaks, specialist pain relief and end of life care for 18 to 25 year olds who currently struggle to find any appropriate support during their final years of life.

Situation

St Oswald's Hospice provides specialist support for children with life threatening conditions. However, when they reach their 18th birthday, we must disharge them and NO-ONE in our region offers appropriate care and support for these young adults. These young people and their families are left without support at a time when their need is greatest.

Solution

These young people need respite within a safe and supportive environment, specialist pain relief and end of life care from a team who know and understand their needs and the needs of their families and friends. With your help, St Oswald’s Hospice will provide this service
