    Situation

    We provide free specialist palliative care for adults in Hartlepool and east Durham diagnosed with cancer, and other diseases like Multiple Sclerosis, Motor Neurone Disease and Parkinson’s Disease. Our range of services meets the needs of local people. Skilled specialist clinical care, social and spiritual care, counselling and information enables to make the most of time they have left with their families. We offer emotional and practical support for the patient’s family.

    Solution

    The new Health and Well Being Centre will be a high quality environment for Day Hospice services. Individual care plans are developed for each patient including treatment and medical reviews with specialist doctors/nurses. Patients and their families can access to complementary therapy to relieve side effects of treatments and de-stress. We want to create a patient centred environment that does not feel or look like a hospital. The new equipment and furniture will be high quality.
