    Situation

    This project aims to support people (young and adult) who are admitted to hospital for alcohol related illnesses or injuries. During their admission, drug and alcohol specialist community workers from EDAS (a community drug & alcohol service) will engage with the patients on the ward, conduct AUDIT (alcohol screening) tools with them and counsel them about their alcohol use. The worker will liaise with community support services and refer the patient, if neccesary, for ongoing support around their drinking after discharge from hospital. At present, patients are being discharged into the community with little or no ongoing support and being re-admitted to hospital at a later date due to the continuation of their alcohol misuse. This project aims to reduce the number of re-admissions, reduce the number of presentations to A & E that are alcohol related and increase the number of young people and adults accessing support in the community for their alcohol use.
