People with dementia, mental health issues, learning difficulties and those who are elderly, are often unable to access quality ‘end of life’ care in the same way as people with cancer. In response to this, Thames Hospicecare (THc) would like to take its expertise in palliative care outside the Hospice by setting up an ongoing training programme for nurses/carers working within a variety of care homes and the community. This innovative project is a creative and cost effective way for THc to expand its services to a much wider group of people than could ever be catered for within its in-patient unit. Ultimately, this project will help ensure that quality 'end of life' care is accessible to a greater number of people.

    Situation

    The aim of the project is to identify and prevent people in Residential Care and Nursing home environments from being moved as they approach the end of their lives. Some of these people will be elderly and many of them will have additional health problems, such as dementia, mental health issues and learning disabilities. To avoid distress, it is often more appropriate for them to remain within an environment which feels safe and familiar. The training programme will teach the theoretical aspects of ‘end of life’ care and this learning will be consolidated with practical hands- on experience gained during organised placements within the Hospice.
