To support and care for patients from East and West Sussex with life limiting illnesses both within a Hospice environment and in their own homes. To allow patients to make the most of the rest of their lives and ultimately to die with dignity in a place of their choice.

Situation
We receive referrals for our care and support from G.Ps and other health care professionals, with around 200 people on caseload at any one time. Last year we cared for over 500 individuals, alongside providing help and advice to their carers and family. This service is provided at no cost to the individual and we receive less than 16% of the funding we require from statutory sources. Our challenge is to source the remaining fund we need to maintain our much needed services.
Solution
We have a wide range of services to help people wth life-limiting illnesses, including our in-patient unit, respite bed, day care centre, community team plus physiotherapy, complementary therapy, welfare and chaplaincy. We treat our patients holistically to provide the best quality of life free from pain, stress and isolation. We continue to support families after bereavement for as long as is needed.
