Situation

The number of deaths occurring in care homes is projected to double in 25 years. Care homes are still unable to support residents with complex care needs leading to residents being inappropriately admitted to hospital. There is an urgent need to train and support Care home staff to help them provide quality end of life care to patients and enable them to die in the place of their choice and not hospital.

Solution

By providing a dedicated specialist palliative care nurse working alongside care homes, improving the continuity and sustainability of the care within those homes. Care homes have asked us to give them confidence around/understanding of symptom and pain control; individualised Advance Care Planning; communication skills on difficult/sensitive conversations with patients and their family members; syringe driver training; symptom control, and recognition/verification of death or dying.
