Care Team - Family Support Worker

We have grown and now provide care and support to 447 children with life shortening conditions and their families, including siblings, across West Yorkshire. Due to this growth we are now in need of a Family Support Worker to continue our support and enable us to reach out to entire families.

Our mission is to say “yes” to children and families. It is not for them to fit to our service, but for us to deliver what they need in the way that works best for them. Forget Me Not Children’s Hospice is about making precious memories for our children and families.


Forget Me Not Children’s Hospice isn't your average hospice! A large proportion of our support is delivered in the homes of our children and families, through our “Hospice at Home” service. Our highly qualified and dedicated Care Team take as many aspects of our care and support directly to our families so that they can remain in their own comfortable and familiar surroundings, and so that the lives of siblings and other family members are kept as normal as possible.
