    Situation

    The majority of people who need of end-of-life care prefer to receive that care at home. They want to remain independent, be in familiar surroundings and spend as much time as possible with friends and family. But, this desire can place a huge burden on their families. The right kind of specialist care and support can help fulfil someone's wish to be cared for at home and help families and carers to cope, making one of life’s hardest challenges, a little easier to bear.

    Solution

    We support people to fulfil their wish for end-of-life care at home. We help patients develop a care plan that reflects their priorities and we provide the specialist nursing care, advice and support to help make their wish possible. We visit them at home, we phone them and their carers to make sure they are okay and we ensure they make the most of all our Hospice services, including our inpatient care, counselling, welfare advice, befriending, spiritual support and complementary therapies.
